Manual Lymphatic Drainage

Lymphedema Treatment
Lymphatic Drainage is a technique that can assist greatly in improving function and appearance to areas of the body affected by Lymphedema.

Before one can look at what Manual lymphatic Drainage is one has to have an understanding of the lymphatic system, what it does and what happens when it stops working.

Function and anatomy:

The lymphatic system has four major functions:

1. To drain excess interstitial fluid.

2. To transport dietary Lipids from the intestines,

3. To facilitate immune response.

4. To transport Proteins from the blood, through the interstitial fluid and into the lymph vessel system.

The Lymphatic system consists of vessels that are similar to those in the circulatory system, beginning with capillaries, which converge to form larger vessels. These resemble veins but have thinner walls and more valves than veins.

Unlike the circulatory system the lymphatic system is interspersed with bean shaped lymph nodes. These act as filters for the lymphatic fluid, and “garrisons” of T and B immune cells.

The immune cells are not created in the lymph nodes but are released from them when an immune response is needed. There are about 600 of these nodes in the body, 160 or so in the neck. Others can be found in the armpits, thorax, abdomen, inguinal area, and the cubital and popliteal fossas.

Also unlike the circulatory system, the lymphatic system has no central “pump” to move fluid around, and relies on muscle pump action to help to move lymphatic fluid.

The difference between lymphatic fluid and interstitial fluid is really a matter of location. Lymphatic fluid is present inside the lymph vessels, Interstitial fluid is present in the tissues of the body.

Signs and Symptoms:

Lymphatic Draininage
Because one of the main functions of the lymphatic system is to move or drain excess interstitial fluid the most obvious sign is dysfunction in the lymphatic system is edema, excess fluid or swelling.

Lymphedema is edema caused by some form of dysfunction in the lymphatic system. Lymphedema is broken down in to two types:

Primary Lymphedema: is defined as lymphedema without an external cause. This can be due to birth defect, or other hereditary factors.

Secondary Lymphedema: has an external cause. This can be trauma to the lymphatic system such as radiation or removal of lymph nodes or blockage of the lymphatic system as occurs with Filariasis (philariasis) a parasitic infection of thread-like nematodes that accumulate in the lymph vessels.

The most common cause of lymphedema in North America is usually caused by cancer treatments, such as the surgical removal of lymph nodes or damage to the lymphatic system from radiation.

It can take many years after trauma has been done to the lymphatic system before noticeable changes occur although lymphedema can develop soon after surgery and be persistent. Often these changes can be misdiagnosed by Doctors unfamiliar with the lymphedema.

Three Progressive Stages to Lymphedema:

Stage one: is a spontaneous reversible lymphedema. The tissue is soft and doughy in consistency, indentations are easy to make – pitting edema may or may not be present. Edema reverses when limb is elevated.

Stage 2: Spontaneous irreversible lymphedema. There is no regression with elevation of limb. Moderate to pronounced fibrosis is present. Indentations are difficult to make. Stemmers sign is present. ( thickened fold of skin at the base of the second toe or second finger that can be gently pinched and lifted.)

Skin alterations (eg Hyperkeratosis, Papillomatiosis, Pigmentation) are also present as a result of long term edema.

Stage 3: Marked Subcutaneous fibrosis or sclerosis, marked skin alterations (eg Hyperkeratosis, Papillomatiosis, Pigmentation) Inter-digital and nail fungal infections also occur because of reduced immune function.

Heath care and Lymphedema:

Unfortunately the Canadian and Provincial health care systems do not provide payment for lymphedema treatments. Sufferers are left in the uncomfortable position of having to pay out of pocket for treatments that may take multiple applications to have significant effect, and shockingly high prices for custom fitted compression garments to keep the edema under control.

Lymphedema treatments can do more than improve the appearance of the affected area. Limbs that are affected by ongoing lymphedema can lose dexterity and function, they are much more likely to suffer from infections, ulcers and the potentially deadly complication of cellulitis.

Lymphedema Treatments:

Lymphatic Drainage Treatments are very gentle and are generally very relaxing for the client. Generally treatments start at the neck. The reasons for this are two-fold.

The first is because some 160 lymph nodes are located in the neck, and also the neck is closest to the terminus of the lymphatic system that empties into the venous return of the circulatory system.

By treating the neck first the `pump is primed` for drainage and the treatment moves from a proximal to distal treatment into the affected areas.

If the area treated is noticeably enlarged by edema the suggested next step in treatment is the application of bandaging.

Compression bandaging is applied in several layers and is used to keep the treated area from re-filling with fluid until the next treatment.

The compression provided by the bandaging can also provide a further “pumping” action as muscles flex and release under the bandages pushing more excess fluid out of the affected area.

Once the area has responded to treatment and reduced to an acceptable size for the client, it is recommended that the client invest in a custom fitted compression garment.

Treatment can be costly. Individual treatments are usually an hour long including application of bandages and are generally charged at the same rate as massage therapy.

However, in order to be successful, treatments must be frequent (once per day) and depending on the severity of the affected area treatments must be repeated for quite a few days and sometimes weeks.

Even if a client has extended health care coverage, that coverage can get used up very quickly leaving the client to pay out of pocket.

While there is a demand for this service, (I get phone calls once a week about it) very few clients have been able to afford treatments.

Choosing your Massage TableWhen looking to purchase your massage table there several things you need to keep in mind and look for before purchasing. So with this in my we have created a Massage Table Checklist for you to go through to help you with your decision.

Massage Table Checklist

 Type of Massage You Do

  1. What type of work do you do? Are doing mobile massage in which you are looking for something on the lighter side and easy to transport from house to house, office to office or sporting event.
  2. Are you in a clinic and looking for a massage table that easily adjustable for height (hydraulic)
  3. Are you working with pregnant woman and need to have breast recesses and adjustable abdominal support.
  4. Perhaps you are looking for something with a thicker plywood top and inverted truss  to provide extra support for sports massage.

Thus it is important to know first and foremost the type of massage you will be doing and where you will be doing it.

Ergonomics for the Massage Therapist

Face it, you need a table that will allow to move freely and easily around it. The table needs to wide enough to accommodate clients comfortably but must be narrow enough to allow proper posture and mechanics while treating. The last thing you want to do is put you back out! Some tables have accessories for wider and taller people and this good to have in your clinic room. You may choose to have a table custom built for you as well.

Here are some guidelines for to help you with you purchase

Your Height

Table Width

5’4 or shorter No wider than 29”
5’4’’ to 5’10’’ 27” to 31” in width
5’10 or taller 27’ to 33’ in width

Tables made by  Earthlite, Oakworks,  Custom Craftworks and Stronglite have tables in various sizes.

Most table tops are flat without  rounded corners.  If you perform multiple services the design of your table top may change.

Client Comfort

You must keep in my mind that your clients want to feel good on the table before you start your treatment. thus it is important to pick a good foam thickness. Generally you are looking at 2.25 to 3 inches . Now if you want more thickness keep in mind that it will add to the weight of your table if you are planning a mobile massage business.

Weight of Massage TableWeight of Masssage Table

You want to make sure that your table is light enough for you to carry around without feeling that you have to go to the weight room. Weight is generally dependent on the size, material and padding of the table.

The weight of your table will vary depending the on frames. Some are made of aluminum which would be lighter than wooden. Carrying weight will vary from 22 to 30 pounds.

Accessories

You may need to have extra cushions, padding and extensions for your table. Remember people come in different sizes and shapes and you may need to make adjustments to your table to accommodate them. Also things like adjustable head rest and arm rests are important too.

Stationary or Portable Massage Table

Will you be traveling with your business? mobile massage is growing in popularity as more people are willing to pay a little extra you to visit them in their home or business.

Perhaps you are setting up in the  downtown core your city. Are  you looking to participate in sporting events for either pre or post sports massage.

You may want both a portable massage table and a stationery table if you are in a clinic or at home.

Value for Price

There is difference in quality of workmanship from table packages that range from $250.00 to over $700.00.  Be aware of “big box” stores promoting massage tables . While they may have great deals on massage tables, be aware of their warranty and durability. you are looking for quality and something that should last at least 10 years.

Obviously you are on a budget so you want to make sure you get your money’s worth. The least expensive table however may give you the most headaches.

Brand Name

Generally most well known manufacturers make quality products. The most well known brands are Custom Craftworks, Earthlite, Master Massage, Oakworks and Stronglite

Durability and Resale Value

It is important that find a massage table that will last you a longtime. you come to a time in your practice that you want to a new table and will sell the one you currently use. your table should be easy to fix or repair.

A quality table should last a long as your career.

Eco – Friendly

You may prefer to purchase from an environmentally friendly company. These tables are generally made from non depleting rainforest hard woods.

Warranty and Customer Service

Regardless if you visit a store or order online you should get a good warranty from the manufacturer. Generally brand name companies stand behind their products. If you are looking to purchase from small independent or local suppliers ask for names of people who have purchased from them. find out if those customers are happy with their purchase.

Make sure you read the find print of their policies. Things you want to know include:

  • costs of shipping for repair?
  • how long for repair?
  • do they send a temporary table for use?
  • how do they treat you on the phone?
  • how helpful are they?

“I love this table! My clients love that it’s heated. The only problem is that the cords get in the way when I’m folding the table. Also, I’ve ripped on the cord at least twice so far, but that’s just be being clumsy. The cord is fairly short so be prepared to buy an extension cord.

The head rest is very sturdy. I like that the table isn’t made of leather. Who wants to lie in a dead animal skin? Be aware that this table does not come with a carrying case. When I bought mine I didn’t think to look for that feature.

Choosing your Career in Massage Therapy

As a student massage therapist getting closer to your end goal of becoming either a registered or licensed massage therapist I’m sure you are still trying to figure out what you want to do.

As with any choices you make there are pros and cons to each.

Massage Therapy Jobs

Spa Massage

There are always spas that are looking to hire massage therapists. If you find a great place you can earn a good salary along with tips.

It is important that you visit the spa, get background information on the spa itself and speak to any of the massage therapists currently there.

There are a variety of spas out there with different specializations. More and more spas are popping up all over as it is a lucrative business venture for the owner.

For example a medical spa is generally more concerned about treating specific ailments, conditions or disease.

Day spas generally focus on taking your stress away and relaxation. They may ask you to incorporate sales for a product line they carry and offer you commission as an incentive.

The spa may offer you a base massage therapy salary with an incentive program and benefits. You need to do your research and ask many questions.

Keep in mind that unless you are in a downtown core, your hours for massage may be all over the place!

You may also find work in a resort spa. Again make sure you are thorough during your interview with many questions.

Location should be important for you as some places may want you to be on call for emergency fill in.

It is vital that find out who does the booking, who is responsible for the sheets and lotion. Generally most spas take care of that for you. This is one those “perks”.

Sports Massage

Sports medicine clinics usually already have one or two massage therapists that specialize in sports massage.

If you are well connected and have experience working in sports it will be easier to find work. You still have to “pay your dues”.

Many massage therapists in sports medicine clinics have put in a lot of volunteer time to be in the clinic they practice in.

It is even more challenging to have massage therapy employment with a semi professional or professional sports team. Semi – professional sports may not have the money to pay you your worth.

Professional sports will pay very good money for your expertise but also expect high quality work in return. One little blunder and you are quickly replaced.

It is for this reason it is a good idea to get yourself certified as a sports massage therapist

Nursing Homes and/or Hospitals

Massage therapy employment in a nursing home is most likely going to be part time. In this situation you may treat either the resident or one one of the family members.

You may have your own little room, you may be out in the lunch room or common area or you may even be in room of the resident.

Payment may be either through the nursing home or the client themselves.

Hospitals are beginning to understand the value of massage therapists on staff not only for their patients but also for their employees.

You may be able to rent out a room for your practice as well as go mobile within the hospital. Paying rent helps in small way to offset some of the hospitals costs.

Furthermore, working in a hospital allows you to create contacts and open up other opportunities!

Fitness or Health Clubs

This one can be tricky. do you rent space or do you have an employment contract. If you decide to leave make sure you take your files with you.

Different cities, regions and provinces / states have different regulation as far as clinic files go. It is important that you understand that before  working in a gym.

Facility owners get very possessive of “their” clientele so make sure you know the rules and regulation before you enter a contract with a gym owner. massage therapy in gym setting

Again in this situation you have a few options: rent, percent split or salary.

you need to know if you get free access to the facility as part of the agreement and will the owner and /or  staff receive complimentary treatment.

You may also want to split your time between a few fitness facilities in your neighborhood. This way you can build a clientele and if you decide to leave you have clients that will see you elsewhere.

Chiropractor’s Office

Most chiropractors like the idea of having a massage therapist on staff. First they feel it complements them in the work they do. Next it helps them cut down their expenses and may even add to their bottom line.

It is vital that you thoroughly go over any agreement or contract with them. As with most facility or other business owners they put in a NON COMPETE CLAUSE.

Some things in an agreement or contract  you should look for:

  • non compete clause -ie if you were to leave to set up your clinic what is distance it has to be away from the clinic
  • time line for notice of leave
  • vacation time

These are just a few things to consider. Take your time going through the contract and if necessary have a lawyer look at it.

You need to cover your assets!

Rent

Are you renting the space and what is covered in your monthly rent – sheets, lotion and or reception. Are you doing your own bookings or is it through their reception?

Percent Split

If you are on percentage split is there a ceiling or is it the percentage agreed too. How many hours do they want you to be there. Are you expected to things that not directly massage related -ie. volunteer work, staff meetings. how does it work for taking time off. remember when you are off either sick, on vacation or taking course there s no money going to the chiropractor.

Do you need to find a replacement?

For both renting and percentage split are you limited in the services you provide. Imagine you pay close to or over $500.00 for a “Hot Stone” massage course  and find out that you are not allowed to use it in your practice.

Does the chiropractor want you push specialty products such as neck pillows or their own nutritional line to your clients. Again make sure what you do is within your scope of practice.

Mobile  Massage

Mobile massage my be considered massage on wheels. For this type of business you need a good solid portable massage table. Massage in office

There are a few different options for this. You could do house calls or go to business or a combination of both.  as previously mentioned you need to get a massage table, linen and lots of gel or lotion for treatments. One of things you need to consider when looking into this option is your car mileage, car insurance and gas bills.

It is possible if you are working in a downtown core to use the local transit. An other option for a mobile business is to base yourself out of downtown office and only make visits within a certain radius. It is possible you could have enough clients in one building.

Either way you need to ask yourself how much traveling are you willing to do each day to be able to pay the bills.

Multidisciplinary Clinic

In this type of setting you will be part of a healthcare facility that also has nutritionists, homeopathic or naturopathic doctors and acupuncturists plus other healthcare related specialties. This is a great opportunity to learn and share information.

Rehabilitation Clinic

Obviously in school you learn treatments for many conditions that range from acute to chronic. This makes you a valuable asset to a rehabilitation center.

Again like anything else you want to do your research on the center itself and see if it will be a good fit for you.

Teaching

Massage therapy instructorDo you have good communication skills. Did you help out your classmates when they got stuck?

If you answered yes to any of these questions you may want to look into teaching at a massage therapy school. Perhaps you may teach your favorite subject.

You must do your research because all massage therapy schools are not the same. All schools each have their own politics and you find this at any place of work.

Here are some things you need to consider:

  • Location – will it cost more in gas than what you receive in pay
  • Subject to teach – do you know the material
  • Support – what kind of support does the school offer you
  • Classroom size- are the classes greater than 10, 20 or 30 students
  • Scheduling -will it interfere with your own private practice
  • Pay – what can you earn and how often and how do you get paid? – cheque or direct transfer to your account?
  • Benefits – do you qualify for benefits or extended health care?

These are just a few things to keep in mind if you want to go in this direction. Teaching is not for everyone; however it is great platform for public speaking!

Massage Therapy for Motor Vehicle Accidents (MVA)

There are clinics that are run by either physiotherapists or chiropractors that treat people who have been in a car accident. Payment for their therapy is through their car insurance. massage therapy -MVA

There are a few challenges with this group of clients. First there is a lot of paper that needs to be filled out and prepared.

Second and probably the most challenging is receiving payment for services rendered. It is possible that you will wait anywhere from 3 to 6 months before getting paid.

If MVA is the source of your income you will find that your payment will significantly change from pay to pay.

Private Practice

It is very common for many massage therapists to have their own private practice. Some people do this straight from the start once they receive their registration or license.

It is more common to begin elsewhere first and create contacts over a few years then start your own private practice. The question is whether or not you want to work out of your house or rent space in a building.

The challenge with having your own practice is staying in the loop with current events. In this situation more than any, you can truly set your own hours.

The only person you have to answer to is the landlord of your property (this can be either a good thing or a bad thing.)

Key Points To Remember

Due your research or due diligence before signing a contract or business agreement. You need to know if you are responsible to building your clientele or is the place of work going to help you.

Find out if there are any additional marketing costs such as paying for advertising in either the yellow pages or online 411.

Are you responsible for helping clean the clinic or is it the owners responsibility.

Are you expected to volunteer at certain functions or become a member of the local chamber of commerce?

Are you or the clinic responsible for business cards?

Are the clinic owners possessive of your client files. If you decide to leave your place of employment who is responsible for your client files?

Is there a specific dress code or uniform /scrubs you need to wear?

Are there weekly / monthly staff meetings and if so is attendance mandatory?

Is there a yearly allowance given for continuing education courses?

If you are on a commission split are there specific rules for time off for either courses or vacation?

These are just a few things you need to keep in mind.

You have choices to make. The field of massage therapy has both amazing personal and financial rewards ready for you.

Do You Suffer From Shoulder Pain?

Shoulder pain is very common in many trades and professions in which there is any over head load on the body.Shoulder pain - over head reaching

Take for example an electrician who may need to replace a ceiling light. He /she must reach overhead to hold the fixture in place with one hand while drilling a screw into the ceiling.

Shoulder pain is also very common with athletes involved in overhead throwing or hitting actions like a pitcher in baseball or spiker in volleyball.

Either way there is consistency in the overhead position of the arm in relation to the shoulder.

Keep in mind there are several types of shoulder injuries that lead to shoulder pain.

Shoulder Impairment

In this case the rotator cuff muscles have impingement due to a mechanical wear against the the acromiom in the suprahumeral space during elevation activities of the arm.

This shoulder impingement generally occurs for of two reasons:

1. Intrinsic Factors

2. Extrinsic Factors

Intrinsic Factors

Intrinsic factors may be vascular (poor circulation) changes in the rotator cuff, changes at the acromio-clavicular joint , changes to the coroco-acromial arch or the humeral head.

Unfortunately if this is the case , surgical intervention may be the only solution  for the problem.

Extrinsic Factors

Extrinsic factors may be related to poor neuro-muscular control , coordination and faulty posture. This definitely can be treated.

Injuries associated with these extrinsic problems would include: supraspinatus tendonitis,  infraspinatus tendonitis, bicipital tendonitis and tendonitis.

Signs and Symptoms of Shoulder Impingement

  • painful arc – pain throug the middle range of raising your arm to the side
  • poor posture
  • -increased curves in the back
  • muscle imbalance -tight or shortened pectoralis muscles , weak and or lengthened lateral rotator muscles of the shoulder and lengthened or weak mid fibres of trapezius and rhomboids as well as shortened or tight latissimus dorsi, subscapularis and  subclavius -this is simialiar to the poor poture that leads to frozen shoulder.

Treatment for Shoulder Impingement

In most cases shoulder impingement syndromes can be treated with shoulder impingement exercises.

From a masage perspective , treating the following muscles to increase muscle length sense:

  • pectoralis major
  • pectoralis minor
  • subscapularis
  • teres major
  • latissimus dorsi
  • subclavius

You will notice that these are the same muscles treated with someone who has frozen shoulder.scapular mobilization for shoulder impingement

In most cases subclavius is a forgotten muscle . This muscle is responsible for allowing the shoulder to get past 150 degrees of abduction. If it remains shortened it will be difficult to regain full range of motion at the shoulder.

Joint play and scapular mobilization is also essential for complete range of motion.

Here is an example of myofascial release of the latissimus dorsi muscle for frozen shoulder but may also be used for shoulder impingment causingpain.

It is important to remebember that shoulder pain may be treated with massage but must also have shoulder impingement exercises to correct muscle imbalances to alleviate shoulder pain.

MYOFASCIAL TRIGGER POINTSMyofascial Trigger Point Release

Myofascial trigger points are painful, hyper-irritable areas in muscle that cause pain, discomfort, stiffness and limited range of motion.  These are commonly known as knots.

Pain may be local to the site or the pain may refer or travel in a specific pattern.  Take for example the referral pattern of the “gluteus” medius muscle “middle buttock”. Its trigger point pain referral may travel down the back of the leg mimicking a disc problem.

WHAT CAUSES MYOFASCIAL TRIGGER POINTS

There are several things that cause or may lead to trigger points

- repetitive movement of a single muscle

- prolonged use of a single muscle

- sedentary lifestyle

- poor posture – neck, between shoulder blades and back effected most

- lifting weights

-emotional stress that lead to muscle tension

How to Treat Myofascial Trigger Points
Myofascial Trigger Point Therapy

The most common way that you are familiar with is ischemic compression. Ischemia means lack of blood supply.

The purpose of ischemic compression is to deliberately increase the blockage of blood to an area so that, upon release, there will be a resurgence of blood. This washes away waste products, supplies necessary oxygen and helps the affected tissue to heal. This increase of blood flow to the area is called a hyperemia.

This type of treatment is deep and specific so it necessary that you warm up the tissues first.

Here is a simple procedure to follow

  1. Effleurage –superficial –deep –superficial
  2. Petrissage
  3. Muscle stripping
  4. Effleurage
  5. Take the relaxed muscle into a stretch until there is a little discomfort.
  6. Press a finger or thumb directly on TP to tolerable pain (7-8 out of 10) –sustain this pressure
  7. As discomfort decreases, increase pressure gradually for 60 -90 seconds.
  8. Add heat and stretch.  The stretch is vital so it may regain memory for its length.

Give at least two days between sessions.  On average 3 to 10 treatments are needed to restore optimal function.

There are other methods such as needling, spray and stretch and even injections.

Even though ischemic compression is popular for trigger points; many trigger points may be released through muscle stripping and/or myofascial release. The key is to warm the muscle to allow for deeper work.
